Mostly surface area. Low-temperature models pack in extra panels and fins so they can shed the same heat from 45 °C water that an old single panel needed 70 °C to deliver. They look much like normal radiators, fit the same wall positions and connect to the pipes you already have.
A fancoil is a slim unit — wall-mounted, floor-standing or hidden in a false ceiling — with a water coil and a quiet fan. In winter it blows warm air within minutes of switching on; in summer the same unit runs chilled water and cools the room.
